Chronic Disease Program
The focus of the Chronic Disease Program is to improve the quality of care for patients with a chronic illness, such as Diabetes, Cardiovascular Disease and Depression.
Living with a chronic disease is easier when you know what to expect. Cross Trails Medical Center works one-on-one with patients to make sure they have the education and skills to manage their chronic disease

Diabetic Foot Exams

Outreach
Cross Trails Medical Center is an active participant in all three communities served, including Bollinger County, Cape Girardeau County, Stoddard County and surrounding communities. Cross Trails Medical Center works with other communities and agencies to help patients receive access to services. A full-time Case Manager is available to patients to help them find needed services.

Medication Assistance Program
A full-time Patient Assistance Coordinator is on staff to help patients complete paperwork for the Medication Assistance Program. Through this program, patients can receive low-cost medications, if they qualify financially.

Show-Me Healthy Women Program
Cross Trails Medical Center is a participant in this important program that provides access to women’s health care. The Show-Me Healthy Women program provides pap smears, cervical exams and mammograms free of charge.

Vaccines for Children The Vaccines for Children program provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay.
