Sliding Fee Discount
Cross Trails Medical Center offers a discounted/sliding fee schedule based on family size and income to all patients. You must qualify financially to be eligible for the sliding fee. No one will be denied access to services due to inability to pay.

Proof of income includes the following:
- Previous year 1040 and W-2 forms
- Last two paycheck stubs
If you have no income, screening for Medicaid eligibility is required within the last six months.
